Assumption of Risk:

I, the minor's parent or legal guardian, understand the nature of the activities my child will be involved in at CAAM Chinese Dance Theater, and the minor's experience and capabilities, and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activities.

Release of Liability:

I, the undersigned, hereby hold CAAM Chinese Dance Theater harmless form liablility for any or all damages and/or expenses that I or my minor child may incure while particpating in the above programs.

Medical Emergencies:

In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold CAAM Chinese Dance Theater and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for CAAM Chinese Dance Theater.

Payment Policies:

Payment may be made by check or via PayPal. There is no additional charge for using PayPal. Payment is due by the 2nd week of classes or a $10.00 late fee will be assessed. Multiple pay plans are also available.

In addition to class tuition, each student family will pay an annual $10.00 registration fee which will be collected in January and a Costume Usage and Maintenance Fee based on the level of the class enrolled in.
